Patient-Centered Medical Home (PCMH) is a concept where patients take an active role in their own health care, working closely with their primary care physicians (PCP) to navigate through the health care system. The PCP coordinates all of their patients' care, manages chronic conditions, tracks all medications and lab results (prescribed by herself for specialists), offers extended office ours and works with other health care professionals in an effort to promote total health care and avoid complications.
